Mobile Mental Health Services for homebound Seniors living in Hayward, Fremont, Newark and Union City.

Services include:

-          In-home assessment of mental health needs

-          Medication support and management

-          Individual and family therapy

-          Assistance in finding services

-          Crisis intervention

There is also the City of Fremont Human Services - Aging and Family Services Division which which serves Union City, Fremont and Newark and they offer:

-          Case management

-          Caregiver Support

-          Senior Peer Counselors

-          Pathways to Positive Aging

-          Community Ambassadors program (87 Ambassadors: 7 ethnic/faith-based groups (Sikh, Muslim, Taiwanese, Latino, Filipino, India Community Center & Centerville Presbyterian)
